Lets say you install a webcam and the app on your iPhone that goes along with it. You can crank up say your Ring app and see the image and sound though the webcam. How or what is the path the app on the iPhone takes from far away from your local wifi and home network, does the packets take to get from the iphone to your home network?

Let me also add that the Internet provider is blocking all ports on your subscription.

I’m sorry. But I’m not sure that I understood your question.
 
Usually on those kind of apps, the app connects to your local network as well and the end device. Usually those apps have their own servers so, as long as all devices are connected to the internet, they can communicate with each other from anywhere.
 
There are other apps that use different technologies like bluetooth, for example, for local control only.
 
To know how a specific app works, it’s best to check with the app creators.
